Breast problems among men are more common than many think, and an Abu Dhabi hospital has devised a novel procedure to address one particular disorder, which is believed to affect more than 50 percent of all boys and men. healthy in the world.

Gynecomastia, a condition that causes enlargement of male breast tissue, is a common concern that negatively affects men’s self-image and can lead to depression and anxiety.

Cleveland Clinic Abu Dhabi (CCAD) has developed a new reconstructive procedure to innovatively improve surgical outcomes for gynecomastia.

The condition has been reported by 30 percent of patients seeking care through CCAD’s breast health program. The hospital, which is part of the M42 network, treats about 100 cases of gynecomastia each year.

While the condition itself is not a risk factor for breast cancer, each patient is initially examined using imaging, and even biopsies if necessary, to ensure that the enlarged breast tissue is not malignant. This is crucial since 0.5 to 1 percent of breast cancer patients worldwide are men.

Causes, treatment of the disorder.

The condition can occur at different times, including birth and puberty, and is most common in men over 50 years of age.

It is caused by hormonal imbalances, steroid or medication use, obesity, or organ malfunction.

If a medication or other substance is causing breast enlargement, the person may need to stop using the medication or switch to a different one. If the cause is disease, the swelling should go away with treatment. If the condition does not resolve within six months or disrupts the individual’s self-confidence, the prescribed course of action is surgical intervention.

During surgery, doctors remove excess breast tissue and skin, perform liposuction, and reposition the nipple-areola complex to a more natural position. However, removal of the tissue often causes a concave depression in the chest, known as a “disfigured appearance,” and can affect the patient’s self-esteem and overall postsurgical results.

A novel technique improves results

Now, CCAD has developed a novel procedure to improve the body contour, overall appearance and outcomes of patients who have chosen surgical removal of enlarged breast tissue.

The innovative technique has been conceptualized and carried out by Dr. Rafal Iskanderian, a staff physician in the CCAD comprehensive breast cancer program.

During the procedure, the thickened breast fold that forms beneath the swollen breast is used to augment the pectoralis muscle, the largest muscle in the chest wall.

This prevents the undesirable disheveled appearance after surgery.

“Traditionally, the abnormal inframammary fold is carefully resected and removed, but this can create an abnormal appearance of the torso. Our novel technique seeks to prevent the unwanted appearance that can result from corrective gynecomastia surgery and can significantly allow young men who opt for the procedure to be confident in their own skin. We have performed the procedure on 30 patients and the results are extremely promising and well received,” said Dr. Iskanderian.

The breast health program provides a comprehensive clinical evaluation for men with gynecomastia. The comprehensive approach includes history, examination, complete blood work, and imaging tests. The integrated process aims to accurately diagnose hormonal imbalances and rule out possible malignancies associated with gynecomastia.
